Responsibilities
- Administer prescribed medications and treatments according to physician orders and facility policy.
- Monitor residents’ health status, perform nursing assessments, document findings, and communicate changes to the care team.
- Collaborate with the interdisciplinary team to update care plans and ensure quality of care.
- Maintain patient safety and infection control standards; respond to changes in condition and emergencies per protocol.
- Provide basic wound care, and assist with activities of daily living as needed within scope of practice.
- Educate residents and families about care plans, medications, and post-discharge or transition planning.
Requirements
- Active registered nurse license (current and in good standing) and eligible to practice in Vermont.
- Basic Life Support (BLS) certification or equivalent is preferred.
- Prior nursing experience in long-term care, geriatrics, or a similar setting is beneficial.
- Associate degree in Nursing (ADN) or higher; BSN preferred.
- Ability to work in a fast-paced LTC environment and adapt to varying shift schedules.
- Strong communication, documentation, and teamwork skills.
