Responsibilities
- Perform initial evaluations and ongoing PT assessments for residents with mobility, balance, gait, strength, and functional limitations.
- Develop, implement, and adjust individualized treatment plans to maximize functional outcomes and minimize fall risk.
- Provide direct patient care, including therapeutic exercise, gait training, manual therapy, and modality-based interventions as appropriate.
- Monitor and document progress, communicate changes to the interdisciplinary team, and participate in discharge planning and goal setting.
- Ensure patient safety, maintain accurate records, and adhere to HIPAA and facility policies.
- Collaborate with nurses, speech therapists, occupational therapists, and other clinicians to support comprehensive resident care.
Requirements
- Active, unrestricted Physical Therapist license in Washington
- Doctor of Physical Therapy (DPT) degree from an accredited program
- CPR/BLS certification
- Prior experience in a skilled nursing facility or long-term care setting preferred
- Ability to work a 14-week travel assignment with guaranteed hours and a 5x8 day shift
- Excellent communication, documentation, and team-collaboration skills
