Responsibilities
- Provide OT interventions and support to residents as part of the rehabilitation team, under supervision as needed.
- Implement, monitor, and adjust individualized treatment plans to improve functional abilities and ADLs.
- Document patient progress accurately in Rehab Optima (NetHealth) and maintain timely, compliant charting.
- Facilitate therapeutic activities and exercises designed to promote independence, safety, and quality of life.
- Collaborate with occupational therapists, physical therapists, nursing staff, and other team members to coordinate care.
- Ensure safety, infection control, and compliance with facility policies and regulatory requirements.
Requirements
- Active California license as a Certified Occupational Therapy Assistant (COTA)
- Skilled Nursing Facility (SNF) experience
- Proficiency with Rehab Optima (NetHealth) electronic medical record systems
- Ability to work during daytime shifts in a SNF setting
- Demonstrated ability to function effectively within a rehab team
