Registered Nurse – Home Health in Gunnison, Colorado. Travel - temporary assignment starting 04/27/2026 for 13 weeks, with guaranteed 36 hours per week and a weekly pay range of $1,737 to $1,821. Responsibilities
- Provide patient care and case management using the nursing process and the individualized Plan of Care for assigned home health/hospice and palliative care patients
- Administer skilled nursing care to patients requiring intermittent professional nursing services in accordance with departmental policies and procedures
- Make the initial evaluation visit, regularly re-evaluate nursing needs, initiate and revise the plan of care
- Furnish services requiring substantial and specialized nursing skill; initiate preventive and rehabilitative nursing procedures
- Prepare clinical and progress notes; coordinate services; inform physician and other personnel of changes in patient condition and needs
- Counsel patients and families in meeting nursing and related needs; participate in in-service programs; supervise and teach other nursing personnel
Requirements
- Active Registered Nurse license in Colorado
- Colorado adult protective services (CAPS) check required
- Ability to provide nursing care within home health/hospice/palliative care settings
- Willingness to work day shift with call requirements (1–2 nights per week and 1–2 weekends per month) and a rotating schedule
- Reliable transportation to support rural Gunnison location
