Responsibilities
- Conduct comprehensive occupational therapy evaluations for residents and develop individualized treatment plans aligned with rehabilitation goals.
- Implement OT interventions targeting functional independence, activities of daily living, and mobility within the skilled nursing care framework.
- Monitor resident progress, adjust goals as needed, and communicate with the interdisciplinary team to coordinate care.
- Document evaluations, treatment sessions, progress notes, and discharge planning in accordance with facility policies and payer requirements.
- Promote safety, ethical practice, and patient-centered care; contribute to discharge planning and transitions within the care continuum.
- Collaborate with nursing, physical therapy, social work, and other team members to optimize resident outcomes and quality of life.

- Ability to work effectively in a long-term care / nursing home setting
- Strong documentation, communication, and collaboration skills
- Comfort working within an interdisciplinary team and adhering to facility policies and regulatory requirements
