Role Specifics and Benefits:
- Responsibilities: Provide holistic nursing care to LTC residents, including routine assessments, timely medication administration, wound care, IV therapy oversight, and acute response as needed. Develop and implement individualized care plans with residents and families; monitor progress, adjust goals, and document outcomes precisely. Support mobility, nutrition, hydration, pain management, fall prevention, and daily routines with an emphasis on safety, dignity, and independence. Collaborate with the interdisciplinary team to ensure seamless care transitions, accurate charting, and clear family communications.
