Responsibilities
- Deliver direct nursing care for a patient or patient group in a home healthcare - hospice setting, applying emergency room nursing principles as appropriate.
- Triage and assess patient needs, monitor status, and adjust care plans in coordination with the interdisciplinary team.
- Administer medications and treatments, manage IV therapy, and respond to clinical changes or emergencies within scope of practice.
- Document all care and interventions in the Epic EMR; communicate clearly with physicians, social workers, and other team members.
- Educate patients and families on disease process, treatment plans, and safety; participate in discharge planning and transitions of care.
- Supervise and direct activities of nursing staff as assigned, ensuring patient safety, privacy, and quality of care.
Requirements
- Licenses/Certifications:
- Registered Nurse (RN) license in the state of practice (California) – active and in good standing
- Basic Life Support (BLS) or CPR certification from an approved provider
- Advanced Cardiovascular Life Support (ACLS) certification – required
- Hospital Fire and Life Safety (HLFS) training – required
- Epic EMR experience – required
- Education: Bachelor’s Degree in Nursing (BSN) preferred
- Experience: At least two years of recent RN experience in the field
- Skills: Ability to work nights and in a home healthcare - hospice environment; strong clinical assessment, critical thinking, and communication skills; ability to collaborate with a multi-disciplinary team
