Responsibilities
- Conduct comprehensive in-hospital case management for assigned patients, emphasizing discharge planning and care coordination
- Collaborate with the multidisciplinary team to develop and implement individualized care plans
- Facilitate safe patient transitions across settings and coordinate post-acute services and community resources
- Monitor patient progress, identify barriers to throughput, and ensure regulatory and payer requirements are met
- Document care decisions in the electronic medical record and participate in floating to sister campuses as needed
Requirements
- Current RN license with Nursys verification on profile
- BLS certification
- Minimum 2 years of Case Management experience
- Education: Accredited nursing degree; or 5 years of case management experience as an alternative
- Ability to float to other campuses as needed
