Travel Registered Nurse Home Health Job
Registered Nurse – Home Health (Travel) in Kaneohe, Hawaii. This travel assignment starts 06/01/2026 for 13 weeks, with 40 guaranteed hours per week. Shift: 5x8 days. Work is in a Medicare-certified home health setting serving the Kaneohe area with coverage across Oahu. This assignment is a Travel - Temporary Administration opportunity.

Responsibilities
- Lead coordination of patient care by collaborating with the multidisciplinary team; act as liaison between patient, family, nurse, physicians, and care providers to optimize outcomes.
- Serve as a consultant on patient items; ensure the interdisciplinary care plan aligns with the patient’s clinical course and covered services; monitor diagnostic testing, treatments, and procedures.
- Oversee and coordinate patient care across disciplines; monitor appropriateness and timeliness of care and continuing care needs.
- Discuss resource utilization and treatment plans with physicians; participate in discharge planning and length-of-stay considerations; address complex cases, frequent ED utilizers, chronic pain, substance use, and housing challenges.
- Collaborate with acute care case managers to ensure ongoing care appropriateness; coordinate transfers to tertiary centers and ensure transfer of information for continuity of treatment.
- Provide oversight and data collection for regulatory/accreditation compliance; conduct focus studies for frequent ED visitors as part of continuous improvement.
Requirements
- Hawaii RN license (active and clear) to practice in good standing
- Active and clear driver’s license and auto insurance
- 1+ year Medicare-certified home health agency experience (required)
- Proficiency with Homecare Home Base and OASIS data collection (required)
- COVID-19 and Flu vaccines on file (no declinations)
- Bachelor’s degree preferred; Case Manager experience preferred
- Ability to work in a home health setting and to travel within the defined service area; own transportation required
