Responsibilities
- Provide direct patient care to residents under the supervision of a registered nurse.
- Administer prescribed medications and treatments according to facility policy and physician orders.
- Monitor vital signs and patient status; document findings in the electronic medical record and communicate changes to the nursing team.
- Assist with activities of daily living and perform CNA duties as needed to support unit operations and patient care.
- Maintain a clean and safe environment; adhere to infection control practices and care plans; participate in interdisciplinary communication and team rounds.
- Be flexible with scheduling to meet unit needs; every other weekend is expected; no on-call or self-scheduling, but opportunities to pick up shifts may arise.

Requirements
- Current active LPN license in New York.
- AHA BLS certification required.
- Minimum 1 year of LTC experience as an LPN.
- Graduation from an approved practical nursing program.
- Ability to work evenings (5x8 schedule) and meet unit needs, including every other weekend.
- Reliable transportation required; public transportation is not available in the area.
- Background checks: 7-year history including NY state check, nationwide checks, county court searches, FACIS3, and sex offender registry within 18 months of start; FACIS-3 & NY state check within 30 days of start.
- LPN is required to cover CNA duties as needed.
