Travel Registered Nurse Home Health Job
Registered Nurse – Home Health Travel assignment in Kaneohe, Hawaii. Start date: 06/15/2026. Duration: 13 weeks. Weekly pay range: $2,270–$2,411 per week. Guaranteed hours: 40 per week. Shift: 5x8 days. This is a travel, temporary home health nursing position in a Medicare-certified setting focused on patient-centered case management and interdisciplinary coordination.

Responsibilities
- Provide comprehensive home health nursing care and participate in the case management process
- Liaise among patient, family, nurses, physicians, and the multidisciplinary team to optimize outcomes
- Lead coordination of patient care across disciplines and monitor timeliness and appropriateness of services
- Ensure the interdisciplinary care plan aligns with the patient’s clinical course, continuing care needs, and covered services
- Discuss resource utilization, treatment plans, estimated length of stay, and discharge planning with physicians
- Focus on complex cases, high-utilization patients, chronic pain, substance use, and homelessness as relevant
- Collaborate with acute care case managers to ensure appropriate ongoing care and coordinate transfers when needed
- Collect data for regulatory and accreditation compliance and support quality improvement efforts
Requirements
- Hawaii RN license (active and in good standing)
- Minimum 1 year of Medicare-certified home health agency experience
- Active driver’s license and current auto insurance
- Experience with Homecare Home Base and OASIS (assessment and documentation)
- COVID-19 and Flu vaccines required; no vaccine declinations accepted
- Bachelor’s degree preferred
- Ability to work in a home healthcare setting and manage episodic, travel-based assignments
- Must have reliable transportation and be able to travel within service areas as assigned
- On-call availability: 6 shifts per month, including weekends
