Responsibilities
- Plan, implement, and monitor occupational therapy interventions for adult residents in a skilled nursing setting.
- Perform and document OT evaluations, establish measurable goals, and contribute to treatment plans in collaboration with the interdisciplinary team.
- Provide therapeutic activities and modalities to improve skills for activities of daily living, mobility, cognition, and safety.
- Educate residents and caregivers on adaptive techniques, carryover programs, and home safety strategies; train staff as appropriate.
- Reassess progress periodically, adjust treatment plans, and participate in discharge planning and transition planning as needed.
- Maintain accurate, timely documentation in electronic medical records and ensure compliance with professional standards and facility policies.
Requirements
- Experience in an occupational therapy setting, with knowledge of standard SNF rehabilitation practices preferred.
- Ability to work in a skilled nursing facility environment and with adult populations.
- Willingness to complete a 13-week travel assignment with the stated start date and guaranteed hours.
- Demonstrated communication and teamwork skills to coordinate with an interdisciplinary care team.
- Eligibility to work in the United States and ability to meet state practice requirements.
