Responsibilities
- Assess residents’ physical therapy needs and create individualized treatment plans
- Provide skilled PT interventions, including therapeutic exercises, gait and balance training, and mobility coaching
- Monitor progress, document sessions, and update care plans in coordination with the care team
- Ensure patient safety during therapy, demonstrate proper technique, and use assistive devices as appropriate
- Communicate clearly with patients, families, and interdisciplinary staff; contribute to discharge planning and goal setting
- Adhere to professional standards, documentation requirements, and facility policies
Requirements
- Doctor of Physical Therapy degree from an accredited program
- Current or eligible Physical Therapy license in New Jersey
- Ability to work in a skilled nursing facility setting
- Willingness to travel for a temporary assignment
- Strong communication and documentation skills
- Experience in geriatrics or SNF environments preferred
