Responsibilities
- Assess patient needs and develop individualized care plans as part of RN case management
- Coordinate hospital-to-post-acute care transfers and transitions, ensuring continuity of care
- Monitor utilization and patient status to support appropriate levels of care
- Collaborate with physicians, social work, and multidisciplinary teams to optimize outcomes
- Conduct discharge planning and provide patient and family education
- Document care plans and progress in the electronic medical record per policy
Requirements
- Active RN license in the state of practice (Oregon) required
- CPR or BLS certification (AH policy or approved equivalent) required
- Valid driver’s license and current automobile insurance; must be 21 years of age or older
- Ability to perform in a hospital-based care management environment on a travel assignment
- Experience in care management or discharge planning is preferred
