Responsibilities
- Provide comprehensive patient care by participating in the development, implementation, revision, and reporting of the case management program.
- Act as liaison among patient, family, nursing staff, physicians, multidisciplinary team, and patient’s healthcare benefactor to optimize outcomes.
- Serve as a consultant to the healthcare team on specific patient items and care coordination needs.
- Coordinate a continuum of care for a defined patient population from pre-admission through post-discharge.
- Ensure patients and families progress efficiently through the course of hospitalization and discharge planning.
- Monitor and report on case management metrics to support care quality and outcomes.
Requirements
- Active Registered Nurse license in California
- Experience in case management or care coordination preferred
- Strong communication and collaboration skills to coordinate care across disciplines
- Ability to work effectively in a fast-paced, inpatient-type care environment
- Comfortable navigating pre-admission through post-discharge processes
