Responsibilities
- Provide direct patient care within the case management framework, coordinating care across the multidisciplinary team.
- Develop, implement, revise, and report the case management plan to optimize patient outcomes.
- Serve as a liaison among the patient, family, nursing staff, physicians, and other care team members to facilitate timely and effective care.
- Act as a consultant to the care team on patient-specific items related to planning, access, and transition of care.
- Coordinate a continuum of care from pre-admission through post-discharge to support efficient progression through the hospital course.
Requirements
- Active Registered Nurse license in the state of California
- Experience in case management or care coordination within an inpatient setting
- Strong communication, collaboration, and documentation skills
- Ability to work in a hospital-based travel assignment and to coordinate complex patient transitions
- Clinical judgment and problem-solving skills to support optimal patient outcomes
