Responsibilities
- Conduct initial evaluations and ongoing assessments of residents to determine PT goals and progress.
- Develop and implement individualized treatment plans focusing on mobility, strength, balance, and functional activities.
- Provide patient-centered therapy sessions, including gait training, range-of-motion exercises, strengthening, and pain management as appropriate.
- Document progress notes, communicate with the multidisciplinary team, and update the plan of care as residents improve or change.
- Monitor safety, maintain infection control standards, and ensure proper use of equipment; contribute to discharge planning and transition planning when appropriate.
- Collaborate with nursing staff and other rehabilitation professionals to optimize rehabilitation outcomes and maintain consistent documentation.
Requirements
- Doctor of Physical Therapy (DPT) degree or equivalent.
- Active Physical Therapist license in the state of New Hampshire.
- Basic Life Support (BLS) certification or equivalent at the time of employment.
- Minimum 1 year of PT experience in a skilled nursing facility or geriatric rehabilitation setting preferred.
- Ability to work a 13-week travel assignment with guaranteed 40 hours per week; eligible to work in the United States.
- Strong communication skills and ability to work as part of a multidisciplinary team; comfortable documenting in an electronic health record.
