Responsibilities
- Provide direct patient care to residents in a long-term care setting per physician orders and individualized care plans.
- Administer medications and monitor for adverse effects; document findings in the medical record.
- Assess residents, monitor changes, and escalate concerns to the care team; adjust care plans as directed.
- Collaborate with an interdisciplinary team (CNAs, therapists, social work, physicians) to coordinate care and ensure timely, safe transitions.
- Maintain safety and infection control standards, perform wound care as prescribed, and adhere to standard precautions.
- Respond to call lights and emergencies; document incidents and outcomes accurately.
Requirements
- Active registered nurse license in Pennsylvania (or eligibility for licensure as required for travel assignments).
- BLS certification required.
- Experience in long-term care or geriatric care preferred.
- Ability to work across all shifts and meet assignment requirements.
- Graduate of an accredited nursing program.
