Responsibilities

- Administer medications and treatments per physician orders and facility policy.
- Monitor residents, perform routine assessments, and document changes in care plans.
- Assist with activities of daily living and mobility, ensuring resident comfort and safety.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Maintain infection control practices and ensure accurate charting.
Requirements
- Active LPN license in Pennsylvania.
- Prior long-term care or skilled nursing experience preferred.
- Ability to work as scheduled with shift flexibility.
- Eligibility to work in the United States.
