Travel Physical Therapist General Job
Physical Therapist (General) position in Bala Cynwyd, Pennsylvania, United States. Travel - Temporary Assignment starting 09/02/2026 for 12 weeks. Weekly pay: $2,505–$2,675 per week. Guaranteed hours: 40 per week. Shift: 8:00 am–4:30 pm (8.5 hours), five days per week. Setting: Home healthcare with a local contract Allied Health program. This role covers the NW Philly and Montgomery County territory; experience in home health is required.

Responsibilities
- Provide physical therapy services to home health patients in the NW Philly and Montgomery County area
- Conduct start of care assessments and periodic revisits per the plan of care
- Develop and implement PT treatment plans with measurable goals; adjust as needed
- Complete documentation in EPIC and manage OASIS data as required
- Collaborate with an interdisciplinary care team to optimize patient outcomes; coordinate with caregivers
- Maintain accurate progress notes and contribute to discharge planning and care transitions
Requirements
- Active physical therapy practice preferred with recent home health experience
- EPIC EMR experience and OASIS knowledge required
- Experience with start of care and revisit assessments
- Ability to work in a home healthcare setting and travel within the specified territory
- Current PT licensure as required by state regulations (format and specifics aligned with Pennsylvania practice)
