Physical Therapist needed for a travel assignment in a long-term care (LTC) / nursing home setting in Bangs, Texas, United States. Start date: 04/27/2026 for a 13-week assignment with 36 hours guaranteed per week on daytime shifts. Weekly pay ranges from $1,906 to $2,001.

Responsibilities
- Perform initial and ongoing physical therapy evaluations for residents in the LTC environment.
- Develop individualized treatment plans to improve mobility, strength, balance, endurance, and functional independence.
- Implement therapy interventions including gait training, ROM, strengthening, neuromuscular reeducation, and functional activities.
- Monitor patient progress, adjust plans as needed, document outcomes, and communicate with the interdisciplinary care team.
- Promote safety, infection control, and adherence to care protocols; participate in discharge planning and home exercise program development.
- Educate residents and families on therapy goals, prognosis, and at-home maintenance; provide guidance on assistive devices and adaptations.
- Document therapy notes in the electronic health record promptly and accurately; collaborate with nursing and medical staff to coordinate care.
- Contribute to quality improvement efforts related to rehabilitation services and participate in ongoing professional development.
