Responsibilities
- Conduct initial PT evaluations and develop individualized treatment plans for residents
- Implement therapeutic interventions, including therapeutic exercises, gait training, balance work, and functional mobility activities
- Provide pain management techniques, ROM, strengthening, and modalities as appropriate to residents’ goals
- Monitor progress, document findings, and communicate with the interdisciplinary care team
- Reassess plans regularly and adjust treatments to maximize safety and outcomes
- Support discharge planning and facilitate transitions to activities of daily living and community re-entry when possible
Requirements
- Active physical therapist license or current eligibility per applicable state regulations
- Experience in a skilled nursing facility or geriatric rehabilitation setting preferred
- Strong documentation and communication skills; ability to work within an interdisciplinary team
- Ability to fulfill the 8:00 a.m.–4:30 p.m. shift and commit to the 13-week assignment
