Responsibilities
- Serve as the primary RN responsible for a patient caseload and lead the interdisciplinary care team.
- Conduct comprehensive assessments and develop care plans, with focus on symptom management, goals of care, and patient comfort.
- Coordinate services across disciplines, document progress, and communicate updates to patients, families, and the care team.
- Educate patients and caregivers on care plans, treatments, and self-management strategies to support informed decisions.
- Advocate for patient-centered goals, monitor for changes in condition, and adjust plans accordingly.
- Ensure quality, safety, and regulatory compliance in all aspects of case management and patient care.

Requirements
- Current, active Registered Nurse license in Colorado (RN)
- Experience in case management or hospice/home health care is preferred
- Strong assessment, critical thinking, and communication skills
- Ability to work a weekday schedule with potential back-up for weekends and on-call duties, as needed for the assignment
