Responsibilities
- Deliver coordinated nursing care for a patient or assigned group of patients according to established standards of care and the nursing process.
- Assess patient conditions, develop and implement care plans, administer medications and treatments per orders, and monitor outcomes.
- Document care accurately and communicate changes to the physician and interdisciplinary team; participate in care coordination with other disciplines.
- Supervise and direct activities of multiple levels of nursing staff, delegating tasks appropriately and ensuring safety and infection-control standards.
- Apply critical thinking, professional judgment, and independent decision-making to address patient needs and escalate concerns as required.
- Provide essential patient and family education to support care continuity in a home hospice setting.
Requirements
- Active RN license to practice in California (or valid multi-state license)
- CPR/BLS certification
- ER or acute care experience preferred
- Ability to work 12-hour night shifts
- Comfort working in a home healthcare – hospice environment and coordinating with an interdisciplinary team
- Effective communication and collaboration skills; reliable with a strong patient-centric approach
- Willingness to participate in a travel assignment and operate within the Los Angeles area as needed
