Responsibilities
- Administer skilled nursing care to patients per physician orders and the established plan of care under RN supervision.
- Assess patient physical, psychosocial, and environmental needs; communicate outcomes to the RN Case Manager.
- Participate in planning, coordinating, and implementing care with the RN to deliver quality, cost-effective patient care.
- Maintain clear, timely documentation of interventions and outcomes; communicate with patient/family and healthcare team.
- Educate patients and families on appropriate self-care techniques; monitor response to care.
- Apply infection control practices in all care activities; ensure safety for staff and patient.
- Assist with admissions, discharges, and transfers as needed; may participate in on-call duties.
- Utilize electronic medical charting and time-keeping software (Homecare Homebase) for documentation.
- Maintain professional boundaries and report unsafe conditions to supervision.
- Read, write, and speak fluent English; uphold attendance expectations.

Requirements
- Current LPN/LVN license in the practicing state (Colorado)
- Current CPR certification
- Minimum of two years’ LPN/LVN experience in a clinical setting; home health or hospice preferred
- Proof of Hepatitis consent/declination and TB vaccination record
- Valid driver’s license and current auto liability insurance
- Experience using Homecare Homebase or similar electronic charting/time-keeping software
- Ability to work in a home-based care setting and coordinate with interdisciplinary teams
- Reliable transportation and ability to provide care in patients’ residences
- Must read, write, and speak English; strong communication skills
