Responsibilities
- Provide direct RN-level nursing care to oncology patients within a home hospice setting, including symptom management, comfort measures, and safety monitoring.
- Administer medications and therapies using the Pyxis system; document all administration and outcomes in the Cerner electronic record system.
- Assess patient status, respond to changes in condition, update care plans, and collaborate with the interdisciplinary team to optimize pain and symptom control.
- Educate patients and families on disease progression, treatment options, medication use, safety, and home care techniques; provide emotional support and discharge planning as appropriate.
- Coordinate care with physicians, case managers, and on-call teams; participate in weekend scheduling per pay period and ensure timely documentation and communication.
- Maintain clinical readiness and adherence to infection prevention, patient safety, and organizational policies in a home-based hospice environment.

Requirements
- Active registered nurse license in California.
- Current Basic Life Support (BLS) certification.
- Experience in med/surg and/or oncology or hospice care is preferred.
- Ability to work night shifts (3x12-hour) and manage care in a home healthcare setting.
- Proficiency with electronic documentation and medication administration systems; ability to operate within a Cerner-like environment.
- Strong clinical assessment skills, effective communication, and teamwork in a high-complexity, home-based care context.
