In this role, you’ll deliver comprehensive direct patient care within long-term care facilities. Core responsibilities include conducting patient assessments, administering medications, monitoring vital signs, managing wounds and chronic conditions, implementing and updating care plans, and communicating clearly with physicians, therapists, and families. You’ll contribute to a collaborative interdisciplinary team, ensuring safe transitions between levels of care, preventing complications, and promoting resident autonomy wherever possible. Your experience will guide you as you mentor and supervise Nursing Assistants, support families with education and empathy, and advocate for residents’ safety, comfort, and dignity.
