Responsibilities
- Administer medications and treatments under physician orders and within scope of practice
- Monitor residents’ health status, perform vitals, and respond to changes promptly
- Assist with activities of daily living and provide comfort, hygiene, and mobility support
- Document assessments, care plans, and progress notes accurately in charting systems
- Collaborate with the interdisciplinary team to coordinate resident care
- Report deviations in condition to the supervising RN and physicians for timely intervention
Requirements
- Active LPN license in good standing in Vermont
- Ability to perform care within a long-term care setting
- Strong communication and documentation skills
- Prior experience in long-term care or skilled nursing preferred
- Comfortable with a travel assignment and guaranteed hours
- Proficient in basic computer/charting and electronic health records
