Responsibilities
- Administer prescribed medications and treatments per care plans and physician orders.
- Monitor residents’ conditions, assess changes, document findings, and promptly report to supervising RN.
- Assist with activities of daily living, mobility, hygiene, and comfort for residents.
- Perform routine nursing tasks per policy (vital signs, wound care, injections, documentation, incident reporting).
- Collaborate with the interdisciplinary team to implement individualized care plans and communicate changes to the appropriate team members.
- Uphold safety, infection control, and patient privacy standards consistent with long-term care expectations.
Requirements
- Active Licensed Practical Nurse license in Vermont.
- LPN diploma from an approved program.
- Basic Life Support (BLS) certification required.
- Relevant experience in long-term care or similar health care settings; travel experience preferred.
- Ability to work the listed shifts (Day and/or Evening) and maintain 40 hours per week.
- Demonstrated ability to follow care plans, document accurately, and collaborate with the care team.
