Travel Physical Therapist (Physical Therapy) in Fairmont, West Virginia. Start date 04/06/2026 for 13 weeks. Weekly pay range $1,907–$1,982 per week. Guaranteed hours: 35 per week. Shift: 08:00–16:30. Setting: long-term care/nursing home.

Responsibilities
- Assess residents’ physical therapy needs and develop individualized treatment plans focused on mobility, strength, balance, and functional independence.
- Implement evidence-based therapies, including therapeutic exercises, gait training, and functional activity progressions.
- Monitor patient progress, document outcomes, adjust plans of care, and communicate changes to the interdisciplinary care team.
- Ensure safety during therapy sessions, maintain a clean and organized treatment area, and use assistive devices appropriately.
- Educate residents and their families on home exercise programs, activity modifications, and strategies to maintain gains after therapy sessions.
- Collaborate with physicians, nursing staff, and other therapists to coordinate care and optimize resident outcomes.
Requirements
- Active PT license (or eligible for licensure) for practice in the applicable jurisdiction
- Experience or interest in geriatric rehabilitation or long-term care settings
- Strong assessment, documentation, and communication skills
- Ability to work independently and as part of a multidisciplinary team
- Comfortable documenting and using electronic medical records
- Commitment to delivering resident-centered care and safety in a long-term care environment
