Responsibilities
- Conduct individualized patient assessments and prioritize data collection based on immediate needs; notify the Charge Nurse and MD of abnormal findings.
- Develop and implement a patient-centered plan of care in collaboration with the healthcare team; perform physician-ordered treatments accurately and timely.
- Administer medications according to facility policy, verify accuracy and ensure proper storage and documentation.
- Perform nursing procedures (e.g., EKGs, blood draws, IV starts) and maintain clean, organized work areas; provide patient hydration and basic comfort measures.
- Document assessment findings, interventions, responses, and progress; communicate clearly with nurses, physicians, and other staff; participate in shift handoffs.
- Respond to emergencies using appropriate protocols; coordinate with staff, families and physicians as needed; ensure safe transfers for procedures or testing.
- Maintain patient confidentiality, deliver care with respect for diversity, and adhere to infection control and safety standards.
- Support ongoing staff education, participate in orientation for new personnel when needed, and assist with department coordination to ensure continuity of care.
Requirements
- Active California LPN license
- Graduation from an approved practical nursing program
- Current BLS (basic life support) certification
- Ability to communicate effectively in English (verbal and written)
- Good physical health and stamina; ability to walk, bend, lift and perform required manual tasks; capable of handling challenging behaviors with professionalism
- Demonstrated strong assessment skills and ability to work as part of a team; commitment to patient safety, confidentiality, and ongoing education
