Responsibilities
- Provide occupation-based therapy services to residents according to individualized plans of care.
- Conduct initial and ongoing functional assessments; set measurable goals and track progress.
- Develop and implement therapy interventions to improve activities of daily living, mobility, strength, and cognitive participation.
- Document evaluations, treatment sessions, progress notes, and discharge plans in the electronic medical record; communicate with the interdisciplinary team.
- Participate in care planning meetings and collaborate with nursing, rehabilitation, and social work to optimize outcomes; support safe transitions.
- Maintain infection control, safety, and patient privacy standards; ensure quality documentation and appropriate billing practices.
Requirements
- Current Certified Occupational Therapy Assistant credential; active Maryland licensure or eligibility for licensure in Maryland.
- Ability to work in a skilled nursing facility setting and on-site day shift; 5x8 schedule.
- Strong communication, documentation, and teamwork skills.
- Prior experience in rehabilitation or skilled nursing settings preferred.
- Willingness to travel for assignments and adapt to new environments.
