Responsibilities
- Deliver coordinated nursing care for an assigned patient group according to established standards and the nursing process.
- Collect and analyze health data, identify care needs, and develop a plan of care to achieve outcomes.
- Implement the plan, coordinate care with other disciplines, and promote a safe, healing environment.
- Monitor progress, evaluate outcomes, and adjust care plans as needed; prioritize patient safety and comfort.
- Supervises and directs the activities of various levels of nursing staff; delegate tasks appropriately and ensure team collaboration.
- Provide education to patients and families; participate in discharge planning and care transitions; communicate clear treatment plans to the treatment team.
Requirements
- Active Registered Nurse (RN) license in California.
- Bachelor’s Degree in Nursing (BSN) preferred.
- At least two years of recent experience in the field; acute/ER exposure preferred.
- Certifications: Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) from an approved vendor; Advanced Cardiovascular Life Support (ACLS) from an approved vendor.
- Hospital Fire and Life Safety (HLFS) training or certification required.
- EPIC electronic medical record experience required.
- Ability to work in a home healthcare - hospice setting; strong critical thinking and independent judgment; effective communication with patients, families, and the care team.
- September start date; night shift availability; there is a 13-week contract length.
