Physical Therapist – Home Health, with a focus in skilled nursing care, is available in Glendive, Montana. The 13-week travel assignment begins 07/06/2026, with 38 guaranteed hours per week on day shifts. Responsibilities
- Perform initial PT evaluations and develop individualized plans of care for residents in the skilled nursing facility setting.
- Implement therapy interventions including therapeutic exercise, gait training, balance activities, and ADL (activities of daily living) training to improve functional mobility.
- Monitor patient progress, adjust treatment plans as needed, and communicate regularly with the interdisciplinary care team.
- Document patient care accurately in the electronic medical record and ensure timely progress notes and coding compliance.
- Educate residents and families on home safety, fall prevention, and strategies to maintain independence and function.
Requirements
- Active Physical Therapist license (state-specific licensure requirements will apply)
- Ability to commit to a 13-week travel assignment with 38 guaranteed hours per week
- Experience or comfort working in a skilled nursing facility setting and/or home health-related care
- Strong written and verbal communication skills; ability to collaborate with an interdisciplinary team
- Ability to adhere to facility policies, documentation standards, and patient safety guidelines
