Responsibilities
- Provide direct RN care to residents in rehab-focused units within a long-term care setting.
- Assess, monitor, and respond to changes in patient condition; document changes and interventions.
- Administer prescribed medications and treatments, and collaborate with the interdisciplinary rehab team (PT/OT/speech) to implement and adjust the plan of care.
- Support mobility, ADLs, wound care, pain management, and safety precautions; participate in discharge planning as appropriate.
- Maintain accurate, timely documentation in the electronic health record; communicate with physicians and family members as needed.
- Ensure compliance with facility policies, safety protocols, and infection control practices; advocate for residents’ dignity and quality of life.

Requirements
- Active Pennsylvania RN license in good standing
- Current Basic Life Support (BLS) certification
- Nursing degree (Diploma, Associate, or BSN) from an accredited program
- Rehab or long-term care experience preferred
- Ability to work the stated evening shift and meet the assignment schedule
