Responsibilities
- Provide direct patient care to residents under the supervision of an RN, including monitoring conditions and reporting changes promptly.
- Administer prescribed medications and treatments in accordance with applicable policies and physician orders; document administration accurately.
- Conduct ongoing assessments, identify potential issues, and communicate findings to the supervising RN to support coordinated care.
- Assist residents with activities of daily living, mobility, hygiene, nutrition, and comfort, prioritizing safety and dignity.
- Maintain accurate records, including charting and documentation, and participate in shift handoffs to ensure continuity of care.
- Adhere to infection prevention and safety protocols; respond to call lights promptly and maintain a clean, safe care environment.

Requirements
- Active Licensed Practical Nurse license in New Mexico.
- Graduation from an approved LPN program.
- Experience in a long-term care or skilled nursing setting is preferred.
- Ability to work the night shift (3x12-hour shifts) and manage a fast-paced LTC environment.
- Strong assessment, communication, and documentation skills.
- Commitment to professional, patient-centered care in a team-based setting.
