Responsibilities
- Administer prescribed medications and treatments per orders and facility policy
- Monitor residents’ health, perform vital signs, document observations, and escalate changes to the RN or physician
- Assist with activities of daily living and mobility; promote safety and comfort
- Participate in care planning with the registered nurse; document progress notes and updates
- Perform basic wound care and routine procedures within scope of practice
- Maintain infection control, safety, and quality of care; collaborate with the interdisciplinary team
Requirements
- Active LPN license in Vermont or eligibility for VT licensure
- LPN diploma from an approved practical nursing program
- Ability to work 40 hours per week, including day and evening shifts
- Proficiency with electronic health records and accurate documentation
- Strong communication and teamwork skills; patient-centered approach
- Long-term care experience preferred but not required; able to meet background check and facility requirements
