Responsibilities
- Provide direct nursing care to patients across Med Surg and hospice/home health needs, including symptom management, medication administration, and patient education.
- Document in Cerner; manage medication distribution using Pyxis; conduct bedside shift reports and communicate with the team using the AIDET framework and iPause.
- Collaborate with tele-staffing for patient coverage; navigate rotating weekends and set patterns with open shifts you can pickup; assist with admissions holds as needed.
- Monitor patient conditions, perform routine assessments, respond to changes, and escalate concerns per protocol to ensure patient safety.
- Coordinate with remote intensivist and on-site hospitalist coverage as described (Intensivist remote; Hospitalist: days on site, nights off site).
- Floats may occur to the ED for admission holds; maintain continuity of care across transitions.
Requirements
- Active Registered Nurse license in California
- Current Basic Life Support (BLS) certification
- 1+ year Med/Surg experience or relevant clinical background; hospice/home health experience preferred
- Associate’s or Bachelor’s degree in Nursing (ASN or BSN)
- Ability to work the listed shift and travel to Tehachapi, CA; comfortable delivering care in a home healthcare - hospice setting
- Familiarity with Cerner and Pyxis is beneficial
