Responsibilities
- Provide direct hospice nursing care in an outpatient setting, including routine visits to facilities and patient homes.
- Complete 4–5 patient visits per day, delivering symptom management, medication administration, and essential comfort measures per care plans.
- Document assessments, interventions, and outcomes in the patient’s chart; communicate findings and plan of care with the interdisciplinary team.
- Travel to patient locations, typically covering 30–50 miles per day; mileage reimbursed per IRS guidelines.
- Educate patients and families about hospice goals, symptom management, advance directives, and end-of-life care, while respecting patient preferences and cultural considerations.
- Ensure compliance with hospice regulations, infection control standards, and safety policies; participate in ongoing quality improvement activities as directed.
Requirements
- Active Massachusetts RN license at the time of submission
- BLS (Basic Life Support) certification
- Experienced hospice nursing background required
- Ability to travel 30–50 miles daily and conduct 4–5 visits per day in outpatient/home settings
- Availability for day shift (8:00 a.m.–4:30 p.m.), Monday through Friday
