Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social and mental functions, while meeting department objectives.
- Deliver individual and group therapy sessions as prescribed, documenting progress and adjusting plans as needed.
- Serve as co-leader in an interdisciplinary approach to patient groups, coordinating care with other team members.
- Document assessments, treatment plans and progress notes in a timely, compliant manner.
- Participate in discharge planning and patient education to support functional goals and safety.
