Physical Therapist – skilled geriatric care travel assignment in Nashua, New Hampshire. Start date 06/29/2026 for a 14-week assignment. Guaranteed 40 hours per week. Responsibilities
- Perform initial and ongoing PT assessments for residents; develop and adjust individualized plans of care.
- Provide therapeutic interventions to improve mobility, strength, balance, gait, ROM, and functional independence.
- Facilitate gait training, transfers, and ADL training; select and supervise appropriate modalities and equipment.
- Document progress and treatment plans in the medical record; communicate regularly with the care team and participate in care conferences.
- Educate residents and families on home exercise programs, safety, and fall prevention; promote steps for discharge planning when appropriate.
- Collaborate with nursing, OT, speech, and rehabilitation aides; ensure adherence to evidence-based guidelines and facility policies; maintain a safe, organized treatment environment.
Requirements
- Active PT license in the United States or eligibility for licensure in New Hampshire.
- Experience or demonstrated interest in geriatric or long-term care settings is preferred.
- Ability to work a 40-hour week with a 08:00–16:30 schedule, five days per week.
- Strong clinical assessment, treatment planning, and documentation skills; proficiency with EMR systems.
- Effective communication and teamwork to collaborate with a multidisciplinary team.
- Eligible for travel assignments and able to commit to a 14-week assignment.
