Responsibilities
- Provide PTA services under supervision per the resident plan of care, focusing on LTAC needs.
- Conduct therapeutic interventions including therapeutic exercises, gait and transfer training, ROM, and modalities as prescribed.
- Monitor patient progress, document outcomes, and communicate updates to the supervising PT and care team.
- Assist with patient positioning, mobility, and safety, while maintaining accurate and compliant medical records.
- Collaborate with a multidisciplinary team to support discharge planning and home exercise programs; participate in care coordination as needed.
- Maintain clinical and safety standards, including infection control and equipment maintenance.
Requirements
- Active Licensed Physical Therapy Assistant (PTA) license in New Hampshire
- Graduation from an accredited PTA program (Associate degree)
- Current CPR/BLS certification
- Ability to work in a LTAC/skilled nursing facility setting
- Preferred: LTAC or skilled nursing experience
