Responsibilities
- Perform initial and ongoing physical therapy evaluations for residents in a geriatric care setting.
- Develop and implement individualized treatment plans to improve mobility, strength, balance, gait, and functional independence.
- Provide therapeutic exercises, gait training, ROM work, and appropriate modalities; adapt plans as residents improve or encounter new needs.
- Educate residents and families on treatment goals, home exercises, and safe functional activity; train on use of assistive devices when indicated.
- Document assessments, goals, progress notes, and discharge planning in the resident’s medical record; communicate with the interprofessional team to support continuity of care.
- Maintain safety, infection control, and patient rights standards; comply with regulatory and payer requirements; support smooth transitions within the care continuum.
Requirements
- Active physical therapy license in Pennsylvania (or eligibility to practice in PA)
- Doctor of Physical Therapy (DPT) or equivalent entry-level degree
- Ability to work in a geriatric/long-term care setting
- Strong communication, documentation, and teamwork skills
- Willingness to participate in a travel temporary assignment and adhere to assignment parameters
