Responsibilities
- Conduct comprehensive PT evaluations for residents and establish measurable goals for mobility, balance, gait, strength, and activities of daily living.
- Design, implement, and monitor individualized treatment plans aligned with resident goals and accepted standards of care.
- Provide evidence-based therapeutic interventions, including gait training, therapeutic exercises, balance activities, and functional mobility techniques.
- Document assessment findings, treatment plans, progress notes, and discharge summaries in the resident’s medical record; communicate progress with the interdisciplinary team.
- Collaborate with nursing, therapy assistants, and other disciplines to coordinate care and support comprehensive resident recovery and safety.
- Educate residents and families on home exercise programs, safety strategies, and steps to maintain progress after discharge.
Requirements
- Active PT license in the United States (or eligibility for licensure to practice physical therapy)
- Ability to work effectively in a skilled nursing facility setting with a geriatric population
- Strong communication and documentation skills; reliable collaboration with an interdisciplinary team
- Willingness to comply with travel assignment requirements and facility policies
