Responsibilities
- Provide direct resident care in a long-term care setting under supervision of a registered nurse
- Administer medications and treatments per care plans and physician orders
- Assess resident status, monitor vital signs, and report changes promptly
- Support activities of daily living (ADLs) and basic nursing tasks (wound care, dressing changes, specimen collection as assigned)
- Document care provided in the electronic health record and communicate with the interdisciplinary team
- Assist with admissions, transfers, and coordination of resident care to ensure continuity
