Responsibilities
- Monitor patients on telemetry, interpret cardiac rhythms, and respond to arrhythmias and alarms promptly.
- Administer medications and therapies as prescribed; collaborate with physicians and the health care team to adjust care plans.
- Maintain accurate, timely documentation in the electronic medical record (Cerner or Epic experience noted as applicable); ensure charting compliance.
- Provide direct patient care, including assessment, symptom management, and safety practices to support comfort and quality of life.
- Educate patients and families on care plans, medications, and discharge planning; coordinate with the interdisciplinary team for smooth transitions.
- Support discharge and home health/hospice services coordination, including equipment and resource referrals as appropriate.
Requirements
- Current California RN license
- BLS and ACLS required
- Hospital Fire and Life Safety certification required
- Cerner and Epic experience: indicate if you have experience; preferred to have familiarity with these EHR systems
- Telemetry nursing experience and comfort working in a home health/hospice setting
- Ability to meet the demands of a 3x12-hour shift pattern and travel assignment
