Responsibilities
- Conduct comprehensive physical therapy evaluations for residents in a skilled nursing setting
- Develop and implement individualized plans of care to improve mobility, strength, balance, and functional independence
- Facilitate timely therapeutic interventions and monitor progress toward goals
- Reassess residents regularly and adjust treatment plans as needed
- Document evaluations, treatment notes, goals, and progress in accordance with policy
- Collaborate with an interdisciplinary care team and participate in discharge planning and care conferences
Requirements
- Ability to work effectively in a long-term care setting
- Strong clinical skills in PT evaluation, treatment planning, and therapy delivery
- Excellent communication with residents, families, and care team members
- Proficiency in timely and accurate documentation
- Flexibility to meet guaranteed hours and the stated shift schedule
