Responsibilities
- Plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions, tailored to home-based patient needs.
- Implement therapy interventions and adjust plans to maximize functional outcomes in a home health/hospice context.
- Serve as a co-leader in interdisciplinary care, collaborating with other team members to coordinate patient care and group sessions.
- Document patient progress, maintain updated care plans, and communicate effectively with the care team, patient, and caregivers.
- Educate patients and families on exercises, safety, and activity modification; provide guidance for sustained home practice.
- Contribute to department objectives by delivering high-quality PT services and adhering to regulatory and safety standards.
