Responsibilities
- Serve as Outpatient/Inpatient Nurse Case Manager for identified high-risk patient groups; focus on specialty clinics needing Case Management attention.
- Assess health and social needs; develop, implement, and monitor care plans; coordinate transitions from admission through discharge; arrange post-acute referrals.
- Collaborate with an interdisciplinary team to assess care plans, monitor progress, and recommend resources to support safe, efficient patient care and appropriate admissions.
- Evaluate ED presentations for medical necessity and care coordination needs; determine admission status and propose outpatient discharge plans with physician approval.
- Coordinate care for patients with abuse, chronic pain, and frequent utilization; facilitate access to mental health resources and navigate available community supports.
- Document progress, respond to emerging client needs, and serve as a resource for nursing, physicians, and ancillary departments on transitional and regulatory issues.

Requirements
- Education: Associate’s or Bachelor’s Degree in Nursing
- Licenses/Certifications: Valid, current, full and unrestricted RN license to practice in any state; Basic Life Support (BLS) certification by American Heart Association throughout employment
- Experience: Minimum three (3) years of clinical nursing experience with at least one (1) year in an Outpatient/Inpatient setting; demonstrated knowledge of case management, contract health, and utilization review processes
- Ability to work in outpatient/inpatient case management within a diverse patient population; comfort addressing mental health needs and social determinants impacting care
- Required to meet orientation cadence (every 2 weeks) and adhere to housing and credentialing requirements
- COVID and influenza vaccination exemptions may be requested via declination form
