Responsibilities
- Provide comprehensive nursing care to patients within a home hospice setting, focusing on symptom management, comfort, and end-of-life care.
- Assess patient condition, respond to changes, and adjust care plans in collaboration with the interdisciplinary team.
- Administer medications and treatments within scope of practice; monitor for adverse reactions and effectiveness.
- Document assessments, interventions, and patient responses in the Cerner FirstNet electronic medical record.
- Educate patients and family members on disease progression, symptom control, and care planning; support decision-making and advance directives.
- Communicate with the broader care team to ensure smooth coordination of services and 24/7 coverage as required.
Requirements
- Ability to work in a home healthcare – hospice setting and participate in a 24/7 coverage model with weekend requirements.
- Proficiency with Cerner FirstNet EMR or demonstrated ability to learn new EMR systems quickly.
- Willingness to commit to a 13-week travel assignment with night shift coverage.
- Strong assessment, clinical judgment, and communication skills appropriate for end-of-life care.
- Capacity to operate effectively in a fast-paced, patient-centered environment and collaborate with an interdisciplinary team.
